Biren, I think the TOEFL requirement varies from school to school. I spoke with an INSEAD student who was from India and he mentioned that at INSEAD, TOEFL was not needed if your language of instruction was English in your under-grad college. But, I dont think that is true with a lot of US schools. When I did my MS in the US, I had to take the TOEFL though the medium of instruction in my school & college was english. It might be better for you to check with the schools you are planning to apply to be on the safer side.
Regarding transcripts, most schools seem to ask transcripts only for degree courses, Master's, Bachelor's, Diploma etc. You will most likely not need transcripts from high school.

Schools rarely seek transcripts from the secondary school level. However, it is common for schools to request transcripts for all post-secondary acdemic coursework even if no degree was awarded.

Thanks Mohit,Gayathri and Hjort for the clarification. A will try to get in touch with couple of school's in the US to check the requirement. I will definitely post my findings in this forum.
