A. Tyres work well (enable braking in the rain) but fail in final goal(bring down number of accidents)

D for me.

It failed in design - as it is designed for better brake, but at the point of impact, the car travelled faster even with better brakes implying that design didn't work.

It failed to achieve its goal -
the goal is "to reduce the number of accidents that occur in the rain", but records show that "cars with news tyres were just as accident-prone as cars with regular tires" => didn't achieve its goal
