Now see what B says:
A Patients are not concerned here – talking about hospitals revenue
B this is what conclusion states
C Some patients’ high income is not point of concern
D if they reduce cost, insurers will also pay less or will not – not effective
E philanthropic donation is OOS

Inferring as little as possible on the draw conclusion is the best idea.

Answer: B

Tip: I find it very useful in wearing the politician hat when answering the conclusion and inference based question. As you may know the politician answers infer as little as possible if at all they answer any question directly.

B is the safest non-assertive and in-scope answer.

D makes an assumption that flies against the premise made in the argument, namely: Insurance companies will pay only at or below cost, which implies that if the hospitals reduce cost, then the insurance payments will also go down correspondingly.

I think the approach should be....stay close to the premise. D assumes too much, which is not stated either explicitly or implicitly in the argument.
