Why not A? B just says it was never used which I agree that it cannot "show" to increase safety. But A shows evidence that the process had flaws.
­B fits with the line of argument. Not used by any other factory & (already mentioned in the argument) not deemed safe enough.
A was a choice as well. I rejected it because
i) B fits better
ii) A mentions problems but does not highlight if these were issues related to safety, which is what the argument is concerned about.

­Take another look at the language in (A):

Quote:
The factory at which the new welding process was FIRST introduced has had several problems associated with the process.
All this tells us is that the FIRST one had problems. Couldn't some other factory have shown the process to be safe? There's no reason to assume they'd be reliant on the first factory alone to demonstrate safety, so (A) doesn't really move the needle here.

I hope that clears things up!

Is it resolving the safety eventually or anything else? There is no clarity on safety aspect in that factory after its introduction in that factory.
(B) The proposed new welding process has not been used in any other factory.
(C) Some of the manufacturing processes currently in use at the factory are not demonstrably safer than the new welding process. - WRONG. Creates more issues than resolves. Anyway it has no impact on the policy implementation aspect.
(D) The safety inspector will not approve any new process that has not been used extensively elsewhere. - WRONG. Just plan statement and nothing else. Already said in the passage.
(E) The proposed new welding process has been used in only one other factory.­ - WRONG. Then what!! It does not say anything further to decide whether to go ahead or not with the implementation. Neither the safety aspect for 1 year is established nor anything on its demonstration is detailed here.
