Hi Sourabh,
You have a solid set of extracurricular activities. It seems you did a lot during your uni days (now 3 years in the past) and have also pursued some solo activities on an on going basis (running, music, salsa etc.). In terms of current volunteer activities you have the NGO. For a non-US applicant you are doing well - a lot of times internationals don't have much in the way of activities outside work because culturally it just isn't as common to be involved in NGOs and the like.
I like your work experience at Sony - my overall impression is that you are involved in strategy/implementation (maybe we could call it operations?) for middle wear. Is that fair?

Short term goals - I have gained exposure to the risk management and disaster recovery though my current project in my company and have gained interest in the same. So want to leverage my technical and financial knowledge and expertise gained from an MBA ,to have a role of a risk manager in a reputed company.
Long term goal - Want to open my own venture specializing in creating improved , efficient and unique risk management models ( still a long shot though).
Having short-term goals that the school can help you achieve is important. I'm a little fuzzy on where you would actually work as a 'risk management and disaster recovery' consultant. Have you seen roles at specific companies that you would be able to reference in an app or interview? If so then we would need to evaluate whether that goal is a NATURAL or believable extension of your past experience + MBA. Without more details I'm going to stay on the fence for now.
Target schools - Columbia, Cornell, Stern , Ross, Tepper.
At Columbia & Stern you'll need to make a compelling case for the school + why NYC. Is there one?, if so an app at these schools would be a possibility to explore. With your GMAT and work experience I feel that Cornell, Ross and Tepper would be reasonable schools to apply to.
My question is given my profile and less than stellar acads are the above schools realistic?
A compelling app (essays, resume, reccos, goals...the works) can help mitigate what you call a 'less than stellar academic record'.
