Profile Review - Trying to compensate for low GPA

Thank you so much for you time. I'm planning on applying in Round 1 this year to business schools and really want to know if I have any kind of chance to get into some of the top programs. I am currently in the healthcare analytics industry and want to stay in the healthcare industry post mba, so programs with strong concentrations in healthcare are important to me.
Age/gender/etc: 27, 28 at matriculation, female, US citizen but ethnically Asian/Indian
Undergraduate: State school, 2.9 gpa. Decision Sciences degree with minors in Management and Physics. Was also part of a National Honors fraternity and president of my class in that fraternity.
undergraduate experience: started at top 50 undergraduate school with a different major, had pretty significant health issues, had to leave school and start over at state school near home. completed new major in two years while working 30+ hours a week as manager at clothing store
GMAT: 760, 48Q 46V 8IR
Work Experience: Healthcare analytics consultant for Fortune 50 Tech company. Have worked there for 4 years now, holding 3 positions (2 promotions) with progressing responsibilities. Haven't directly managed people but have managed projects. I'm not sure how much more detail I need to provide about my projects/wins
Extras: member of Women's BCG at company, holding a leadership position in the steering committee. Helped plan nationwide girls in STEM event
Created and managed company kickball team in local intramural league through several seasons

passionate about team building and creating a positive work environment. planned happy hours, new employee lunches, pot lucks, retirement celebrations, etc for entire applied analytics team
avid weightlifter, maintains and improves my health both mentally and physically
Future goals: Stay in healthcare consulting space with more of an emphasis on strategy and public health

I think that my track record in a very quantitative/analytical job shows that I'm capable of handling the course load of an MBA but I do know that my gpa is very low. What can I do to improve my chances?
