For most schools, is it okay to address multiple things within the same 'optional' essay? I.E. both why there is a gap in work history or why a certain score/GPA was not as high as desired? Is there some smooth way to handle/transition between the two or is it okay to switch topics if including multiple "additional information" topics?

Yes this is ok. If there is no connection you can just separate this by paragraphs.

Totally fine and most schools welcome this if you have multiple points that need clarification. A real transition is not totally necessary usually just separating the disparate thoughts in different paragraphs should do the trick
