Subject of the clause is The boundaries which is plural, hence have will be the correct form of verb. additionally the presence of and indicates parallelism must be maintained, and additionally we have a list in the non-underlined portion of the sentence. Thus parallelism must be maintained with the list

Clearly 1, and 2 are not parallel
B) Correct. lets look the parallelism

2) The boundaries of the Pleasant Valley School District will continue to include the small cities of Millerton, Cedarville, Granite, and Homersfield.
its maintained, and intended meaning is clear
