A.now at a lower rate than
C.now a lower rate than
The infant mortality rate .. is now at a lower rate than --> redundant repetition

D.presently lower than it was
E.presently lower than the rate was
"it" is incorrect because we compare with similar rate in the past, not the same rate. relative clause "that " should be used
"was" past tense is incorrect. it should be replaced with present perfect " has been" because of "before".
"presently" is not an idiomatic way to say "now". I can't say it is incorrect, but if other choices use "now", "presently" shall be avoided.

B is concise and straight forward.
