Grammatically I don't see any error in A and B. Both are correct in their construction.
Even I also seek clarification for A over B.

Study the examples given below.

We can express the same idea without if.

Had I known his real motive, I wouldn’t have supported him
.

In B, 'If' clause is unnecessary, though it's not incorrect, A is short and precise.

The option B is not conditional , for conditional checkup try reversing the sentence by adding 'if' in second part. If sentence works it is conditional .
For eg : - The physicians explained that if the patient had known the warning signs of cancer, he would have come in earlier --> make sense.
Now reverse : - the physicians explained that if the patient had come earlier , he would have known the warnign signs of cancer --? doesnt makes sense , so A

Official Explanation

(C) and (E) are incorrect because the clause expressing past condition contrary to fact requires past-perfect subjunctive (“had known”).

(D) is incorrect because the clause stating a possible conclusion requires the perfect form of a modal auxiliary (“would have come”).

(A) and (B) both use the correct verb forms in both clauses but (B) adds “if ”; this word is superfluous and alters the word order required by Standard Written English.

The correct answer is (A).

A and B are both correct. However, A is punchier and cleaner. It's perfectly acceptable to place the 'effect' (then) part of the if/then construction prior to the 'condition' (if).
