Guitarists are not heavy metal or is made up of one. So heavy metal is wrong adjective for the noun guitarists. We can directly eliminate A, C and D
Among B and E, Let say E is correct, Some guitarists who play heavy metal are rather skillful, but, ironically, so loudlythat a listener may have difficulty appreciating it.
The guitarists who play heavy metal music, play that loudly enough that listeners may have difficulty in Appreciating their work. But when the work happened? They say the guitarists who play heavy metal but verb is not in action to determine the consistency of guitarists music and listeners distortment so E can't be the answer. Answer is B
Some guitarists who play heavy metal are rather skillful, but, ironically, so loudly
The underline potion is adjective, so if you read the sentence without adjective it says that guitarists are skillful and loudly that is not the intended meaning of the sentence.
If, similarly look at option B by removing the fluff . It clearly says that "the guitarists action (do so) is so skillful and loudly "which clearly removes the error present in option E.

In the case of some heavy metal guitarists(when they play it), it is rather skillful but so loud...what? what is it, and what's loud? "when they play it should" lead us to "it" referring "guitar" which has not been mentioned. That means guitar itself is skillful? logically, it sounds like to me that "D" is trying to say that cases are loud, but it's described as 'it', not even they/them.
B. Some guitarists, when playing heavy metal, do so rather skillfully but, ironically, so loudly is only choice left after PoE.
