The reason for which Grace O'Malley, who was an Irish noblewoman who lived in the 16th century, received the title "The Sea Queen" was because of her formidable control of ship routes and her aggressive and courageous attacks on both English and Irish noblemen's fortresses.

(a) The reason for which Grace O'Malley, who was an Irish noblewoman who lived in the 16th century, received the title "The Sea Queen" was because of
reason and because mean the same thing so redundancy
(b) The reason for which Grace O'Malley, an Irish noblewoman who lived in the 16th century, received the title "The Sea Queen" was
correct
(c) The reason for which Grace O'Malley, an Irish noblewoman who has lived in the 16th century, received the title "The Sea Queen" was
HAS lived is present perfect and is used to talk of ongoing things.. illogical here. she is still not living there, event is over
(d) Grace O'Malley, an Irish noblewoman who lived in the 16th century and received the title "The Sea Queen" was
awkward construction
(e) The reason that Grace O'Malley, an Irish noblewoman who lived in the 16th century, received the title "The Sea Queen" was because of
again redundancy and wrong usage of that..
