The original sentence says that it will create. (B) says that they regularly do create. Also, use of "create" is wrong. Should be creates as it talks about the "combination".

C. The combination of an aging population and adverse behaviors such as obesity and smoking creates epidemics of diabetes, heart failure, and other diseases that lasts a lifetime and are difficult to treat. => same as above. Creates instead of will create.

D. The combination of an aging population and adverse behaviors such as obesity and smoking will create epidemics of diabetes, heart failure, and other diseases that last a lifetime and are difficult to treat. => difficulty of treating and lasting a lifetime correctly modify the diseases. Will create preserves meaning. Create correctly modifies the combination.

E. The combination of an aging population and adverse behaviors like obesity and smoking create epidemics of diabetes, heart failure, and other diseases that lasts a lifetime and are difficult to treat. => incomplete sentence stub.

And we can eliminate by using Such as/ Like...

It has to be 'Such as' because obesity and smoking are given as an examples in the sentence. So B and E are out...

'such as' vs 'like' split

such as is used for examples therefore , choice B & E out.

C is out as it uses lasts singular verb form .

between A and D , in A 'which' refers to lifetime (wrong in meaning)

D is better constructed , correct choice.

Can someone please explain what is the subject -verb pair in this question. I thought combination is the subject and that's singular so CREATES would be the verb.

Combination of aging and adverse lifestyle ( i thougth of aging and lifestyle are in prepositional phrase)
Please clear this doubt.

As an absolute rule for subject-verb agreement always remember the moment "and" is used to present a list "x and y" ,plurality is introduced in the sentence. Any sentence having "x, y and z" or "x and y" will always be plural. However do take care to see the function of "and" because apart from introducing list "and" is also used as a conjunction.
