An anthology of poetry by Anne Bradstreet, a Puritan settler in the North American colonies, was published in 1650, the first book of poetry by a woman in the colonies and it was only the second published by a Puritan.
(A) woman in the colonies and it was only the second published by a Puritan
(B) woman in the colonies and only the second published by a Puritan
(C) colonial woman and it was only the second that a Puritan published
(D) colonial woman and the second one only published by a Puritan
(E) woman in the colonies and the second one only that a Puritan published
A and C not parallel because after 'and' we have S+V, while before just a noun phrase. D and E are wrong because they have wrong placement of 'only'. In both cases those 'only' changes meaning of the sentence when in one case suggests that she published only two books (she may have published 2 or more - we don't know), the other suggests that she only published, as if no other activities were taken by her, but this is nonsence. Therefore, we can eliminate A,C,D, and E. Only B has proper parallelism and right placement of 'only. B is answer.

the red part above breaks the parallelism of the sentence. Both of these modify "An anthology of poetry" but this is not clear in the above structure. The "it" is redundant and unnecessary in the above structure.
A and C are out because of the parallelism issue stated above
D and E have an illogical meaning error. The author does not want to say that the second one was only published by a puritan...It's totally illogical, given the context of the sentence. Even E has a similar Meaning error.
IMO Only B communicates the intended meaning in the grammatically correct manner.
Hence, B must be the correct answer

Here, our biggest clue in finding the correct answer is in connecting word 'and'. LHS and RHS must be parallel but in A and C it is not because LHS has just noun phrase while RHS has clause. D and E can be eliminated for two reasons, one is being 'one' that is redundant and confusing (we do not know one refers to what) and place of 'only', which in both cases of D and E modifies wrong word. It must modify 'second' not 'published'. Given all, I choose B.
