The term Socialism incorporates a broad spectrum of economic organization theories but is often associated either with specific communist regimes that have had a totalitarian governmental structure.

Concept tested here is parallisim. For the marker EITHER we need a " Either..x.. OR...y"

A. specific communist regimes that have had a; there is no Or we can quickly eliminate
B. a specific communist regime that has had a; there is no Or we can quickly eliminate
C. specific communist regimes or with a ; This completes the parallelism and makes sense to describe what socialism is associated with.
D. specific regimes or with a communist and ; This completes the parallelism but does not make sense
E. a specific communist regime or a ; this contains an or but breaks parallelism by not having a with.

Its C IMO
please correct me if I'm wrong!

In structure : Either X or Y.
X begins with "with".
Thus, Y too begin with "with"

Only choices C and D meet this criteria.

The term socialism is often associated either with
C. specific communist regimes or with a totalitarian governmental structure.
D. specific regimes or with a communist and totalitarian governmental structure.

Both the sentences are grammatically right.

We need to visit the meaning aspect of the sentences (go back to original sentence). The non-underlined parts don't give a clue to pick C or D.
The last resort is to visit the underlined part of choice A, which says we need "communist regimes" and not just any regimes. Hence, I will pick C.

(I don't think any official GMAT problem will force us to visit the underlined part of the original sentence to grasp the meaning).

2. Intended meaning

Original sentence:

Eliminate A, and B -
- idiom Error (Either X ..Or ..Y)
- usage of tense "had" indicates process/action completed, and "that" usages for making had as an essential information of sentence - brings flaw to the intended meaning of the sentence.
- Option B, regime (singular) not parallel to (structure) plural

B. a specific communist regime that has had a

C. specific communist regimes or with a
Correct

D. specific regimes or with a communist and
Wrong: In Option D.
Regimes, is a Noun, which means a government, especially an authoritarian one.
Coummunist, is a Adjective which modify noun - Regimes, with option D, it indicates regime with person communist, this makes intended meaning of sentence error. It must be "specific communist regime" as one identity.
In end "Add" try to give another additional information about "totalitarian governmental structure" and makes it third additional part of character, which "organization theories" is associated with in the sentence.
