University grades: self-reporting form question

Kudos
Hello all,
I'm working on filling in the excel template for self-reporting university grades, and would be thankful for advice on 2 questions
1) The template indicates that I need to provide the semester / term in which each course was taken. My diploma supplement doesn't provide this info; what would be the best way to fill in this form? 2) Can I provide self-calculated GPA & class rank (my school doesn't calculate these)?
Thanks!
PS. Theoretically, I could try to allocate each class to a particular semester from my memory, but it's (1) time-consuming; and (2) not precise, as I had some classes for 2 semesters, for example, but with only 1 grade
