My designation at my current organisation, and which will be shown in the relieving letter would be Senior Business System Analyst. This role is frequently referred to as an Assistant Manager internally in the organization.
In my applications, I would like to pin down my story as an Assistant Manager. One big reason for this because my role is a techno-functional mix, closer to the business side of things. However, the Senior Business System Analyst would give it a perception of an IT related role.
Is it safe enough to write my applications and CV using the Assistant Manager title ? Will there be any issues during the background check ?

It depends on what country you are from (e.g. the background check may not reveal enough details in some locations). It can go a few ways. One of which may not be flattering. It is a risk. In the US, the title will definitely surface in the background check and in the LOR. It would be somewhat obvious and slightly of an irritation for the adcom to learn that and you will have to explain why you provided a role that is not on your work email signature, job description, or LinkedIn Profile. If you are not from the US, you should explain in the optional essay how your role is much more than what your job title entails or perhaps drop it in one of the essays (if it fits naturally).
Another option you have is to put your title on the resume as Senior Business System Analyst (Assistant Branch/Company/Country Manager) as your role or see if you can get the title changed in the company and what it would take.
