Obviously SC is your crux. I'd suggest working on basic grammar skills by reading grammar content.

There's our Verbal Guide (gmat-grammar-ultimate-by-gmat-club-102387.html)
The Manhattan GMAT books
Total GMAT Verbal (https://www.totalgmatverbal.com/)
Veritas

My guess is you "think" in improper grammar and gravitate to choices you are comfortable with, but are incorrect.

I had a similar problem. The way I got over it was I forced myself to start thinking in proper grammar (correcting my thoughts (for a company: its, not their. For everyone, his and her not their, etc.).

Going through content/doing problems alone isn't enough in these cases. You must identify WHY are answering problems incorrectly. When reading content or read explanations, the typical response is "mmhmmm, that makes sense", but for whatever reason, still unable to apply it.

This is why we stress keeping an error log and identifying the "why" aspect to answering problems incorrectly.
