Ok. The situation is like this:
Whenever I practice questions under test environment and simultaneously in a row, I am hitting an accuracy of around 70%. However, when I solve the same questions again after say half an hour or so, I can solve over 95% of all these questions. How do I bring this efficiency into my test mode? I feel like I know most of the concepts yet my test scores don't reflect my true prep level. How do I counter this? Has anyone else faced this peculiar situation?

This isn't unusual at all.
Taking the GMAT is much harder than just
doing the questions. The questions themselves often aren't
that hard - what's hard is doing them under pressure, without a break, with a very limited amount of time.
Keep practicing under test conditions, because that's what you'll have to do on test day! You're doing the right thing, and what you're observing is normal. You need to get better at doing a lot of questions that you haven't seen before, without a break. That's something you can improve via practice.
If anxiety is a factor in doing worse under test conditions - and it is, for most of us - try some anxiety reduction techniques, such as taking a deep breath before you start every problem, and using positive self-talk ('you can do this!').
