Given your background, you really should consider EMBA if you want to go to a school in the US. If you look at the average ages of each class, you really are significantly older than most people. While you have a fascinating background, I'm not sure the AdCom at the Top 50 schools are going to see your background as appropriate for the full-time MBA.
Again, it's not impossible (there are a handful of examples where you see someone 35+), but you should definitely attend a few information sessions of schools on your list and try to get an honest answer from the AdComs if they would even consider you. You really don't want to go through this entire process without knowing that ahead of time or you could be sorely disappointed.
Have you considered the EMBA? Would you be able to work in the US/CAN while obtaining your degree?
