A. 'It' in the phrase It replaces it gives the meaning that the failed point replaces itself, which does not makes sense.
B. If then construction is Proper (If + Simple Present, then + Simple Present) and meaning is clear. - Correct choice
C. Changes the meaning - First, Often is not equal to sometimes and second the pronoun 'they' in they replace do not have an antecedent.
D. Meaning change.
E. Construction is awkward without the preposition at and here Often is modifying replaced thus changing the meaning.
