The tomb along with the monument has survived not the other way around that's tomb installation after the earthquake meaning error therefore out

Simple past is absolutely fine, usage of had is unnecessary therefore out

I had trouble with this answer choice and ultimately went with A even though it was awkward. Wouldn't "that survived" and "remains" violate the || rule? Past vs Present. Shouldn't "remains" be "remained"?

Because "medieval earthquakes" happened in the past, the survival is also a past event. It is thus correct to use "survived".
As for the "remains...to this day", it is indeed a state with current resonance. The simple present tense makes sense.

Parallelism does not require the use of the same verb tense. In practice, parallelism offers important clues about verb tenses, and, without good reason, it makes sense to apply the same verb tense consistently to the paralleled parts. But do not apply such "rule-of-thumb" blindly. Another example: Tom was born in Paris but lives in Tokyo now.
