Why is A wrong? To which has been added, here which refers to dish(singular) and has is correctly used for the subject dish.

C. to which shellfish, fresh tomatoes, wine sauce, and other savory ingredients have been added
D. with shellfish, fresh tomatoes, wine sauce, and other savory ingredients having been added to it
E. and, in addition, shellfish, fresh tomatoes, wine sauce, and other savory ingredients are added
Option A "has been" is used, but there are multiple item which were added like, tomatoes, sauce etc. which make it wrong where in option C it rightly captures this point by using have been.
