Why is A wrong? To which has been added, here which refers to dish(singular) and has is correctly used for the subject dish.

I agree. Why is A wrong? Which clearly refers to the soup.

Hey! Let's look at the question once again.

Cioppino is a rich seafood stew developed in Italy by Genoan fishermen who migrated to California in the mid-1800s; the dish is essentially a regional fish soup to which has been added shellfish, fresh tomatoes, wine sauce, and other savory ingredients.

A. to which has been added shellfish, fresh tomatoes, wine sauce, and other savory ingredients
B. added to which is shellfish, fresh tomatoes, wine sauce, and other savory ingredients
C. to which shellfish, fresh tomatoes, wine sauce, and other savory ingredients have been added
D. with shellfish, fresh tomatoes, wine sauce, and other savory ingredients having been added to it
E. and, in addition, shellfish, fresh tomatoes, wine sauce, and other savory ingredients are added

Option A "has been" is used, but there are multiple item which were added like, tomatoes, sauce etc. which make it wrong where in option C it rightly captures this point by using have been.

One follow up question - A has the subject verb agreement error from your explanation. Therefore, if the phrase is changed to "to which have been blabla.. and other savory ingredients" would A be right?

Attention, there is a to before which, so the order of the clause is shellfish, fresh tomatoes, wine sauce, and other savory ingredients has been added to. so the has is wrong.
