D for me.

It failed in design - as it is designed for better brake, but at the point of impact, the car travelled faster even with better brakes implying that design didn't work.

It failed to achieve its goal -
the goal is "to reduce the number of accidents that occur in the rain", but records show that "cars with news tyres were just as accident-prone as cars with regular tires" => didn't achieve its goal

A. Tyres work well (enable braking in the rain) but fail in final goal(bring down number of accidents)

The new type of tires were designed primarily with an aim of enabling the cars to brake better in the rain. There by the manufacturer aimed to achieve its final goal of reducing the number of accidents that occur in the rain. So based on the study, it was found that the tires are in fact travelling at faster speeds during the point of impact (this flaws the primary design) and also that the number of accidents are not reduced (this proves final goal is not achieved).
