well in my opinion the argument concludes --he too is to be blamed (not just contractor) because X and should have done Y.
A is the combination of X and Y, mere reiteration of what is stated. Nowhere mentioned that he didn't planned. what if he planned but still failed.
so D is the correct choice that states clearly that why he is to be blamed? since he is accountable for the mistakes of those, whom he supervises
