The cost of a taxi fare consists of an initial fee of $3 plus an additional $0.75 per mile in the city and $0.40 per mile outside of the city. What is the cost of a taxi ride from the airport to the convention center?
(1) The convention center is three miles outside of the city and eleven miles from the airport.

Breaking Down the Info:
We would like to know the number of miles in the city and the number of miles outside of the city.
Statement 1 Alone:
Even if we assume the airport is outside of the city, we don't know if we will ever cross the city within these 11 miles. Therefore statement 1 alone is insufficient.
Statement 1 Alone:
Again, we're unable to tell how much of the travel would be within the city. We don't even know if the hotel is within the city or not. Then this statement is basically useless, so statement 2 alone is insufficient.
Both Statements Combined:
We have 11 miles to break down. We could have all 11 miles outside the city so that the hotel in statement B would split the 11 miles into a 7+4. It is also possible that we have 2 miles inside the city and 9 miles outside the city. Again we do a 7 + 4 split, but each section would have 1 mile inside the city.
Thus the cost of a taxi ride is cannot be confirmed. Both statements combined is insufficient.
