we can use since in two ways 1. time marker 2. to give some reasoning ( like because)
Example: 1. since 1947 , India have developed substantially ---present perfect 2. I will not ride the bike , since i have serious bone injuries---reasoning.

1. When using present perfect, to denote the time we use since.
2. since is also used to mean "for the reason that" or "because"
so here - health system started offering visits BECAUSE/SINCE it negotiated payments from insurers.
3. since is also used to mean "ago". that is long ago or say long since.
Hope the meaning is clear.
