My answer is A.

B and D can be eliminated because you don´t use THAT when the modified noun (green "bloom") is the object of the clause.
C can be eliminated from the intended meaning of the sentence. it must be "Aerogu,...,and is produced"
between A and E, I choose A because "which" is used to refer to the preceding noun. In this case the second "which" in the sentence do not has a preceding noun.

Aerugo, also known as verdigris, is the green "bloom" visible on many copper items, and is produced, like iron rust, over the course of time by the exposure of the metal to the oxygen in the atmosphere.

a) visible on many copper items, and is produced -- fine - hold it.
b) that is visible on many copper items, and which produces -- parallelism error -- we can not have that || which clause.
c) visible on many copper items, and produces -- produces (active voice) -- Aerugo is not producing anything here, instead it is being produced by something.
d) that is visible on many copper items, and that produces -- same as C -- active voice issue.
e) which is visible on many copper items, and which is produced. -- by keeping two clause parallel at same level. we are introducing ambiguity that "by the exposure ... blah blah" is making bloom visible as well, but that is not the case here; passive part only required for "is produced".

Can somebody suggest what ''visible on many....'' refers or modifies to ? Bloom i.e. immediate noun or Aerugo ?

Please let me know what are the Modifiers rule for such constructions.

It's a bit debatable. Here bloom refers to Aerugo; therefore modifier to bloom is indirectly referring to Aerugo. Such reference is acceptable.
A should be the answer.
