I'm sorry to hear that Test Day did not turn out better. A 640/Q44 is still a solid Score though - and it might be enough to get you into your first-choice School. In addition, raising a 400 to a 640 is a significant achievement. Before we discuss the data in your ESR (which appears to be missing one page of the data), it would help if you could provide a bit more information on how you've been studying and your goals:
Studies:
1) Over the last 3 months, how many hours do you typically study each week?
2) What 'brands' of CATs have you used during your studies?
3) You listed just 3 CAT Scores. On what dates did you take EACH of your CATs/mocks and how did you score on EACH (including the Quant and Verbal Scaled Scores for EACH)?
Goals:
4) Is your overall goal score 700 or something else?
5) When are you planning to apply to Business School and what Schools are you planning to apply to?

1) I have actually logged every hour spent studying since January (358 hours on Quant, 168 on Verbal; attached).
2) I have only used GMAC GMAT Prep tests
3) Dates for CAT are as follows: 26 Sep - 660 (Q47, V35), 17 Oct - 640 (Q42, V36), 24 Oct - 710 (Q48, V38, but I did get some familiar questions so I know this score is inflated)
4) Yes, my goal is 710 or above.
5) Applying in R1 next year, so I have ~10 months left to get my 710. Applying to M7, LBS, INSEAD, Judge and Said. Unfortunately, a 640 will not be enough.
6) I have reuploaded the ESR with all 10 pages.
