Hey All,
I'm finishing up my applications and I have struggled with the very general question reflected on applications such as Booth, Sloan, and Yale.
"Were you ever the subject of a disciplinary action at a prior academic institution?"
I cannot determine if I need to disclose the fact that during my undergraduate studies I was put on disciplinary probation because there was underage drinking in my room the second week of college. My undergraduate university was very strict so that was essentially the first status my school issued, while other schools may issue a warning. Either way this is expunged on my school record and I've had no other issues. I'm guessing I should just disclose it to be safe but I'm not sure if there was a severity level they're looking for with this question?

Does it open a box/field area that allows you to enter details and provide more information?
I am a bit concerned with disclosing as there may be a blanket rule that basically immediately dings you or puts you into a "problematic" category of applicants. It would be unfair and foolish of them but it has happened when applicants have chosen to disclose a bit too much. A friend of mine was basically cut from getting a job of his dreams because he was honest and disclosed an isolated recreational drug use incident from many years ago. I can't tell you his feelings and the shock. He has spent a number of years working towards the goal and it all just went poof. Different setting so hopefully this is not anything that will hurt you so long after.

Thanks for the quick response on this! It does provide a box to explain on all the applications, and honestly I feel comfortable walking through this. I'm just not sure the scope of the question as they make it so broad, I don't want to not disclose it and then technically have violated each applications honor code.
To actually access my student record that was expunged I would have to sign a consent form which I'm not sure is required by the top programs. Any clarity around these lines would be helpful as well!
