the way I figured out:

these 3 options correct the SV error by introducing "is"

C) flock have arrived from the north and is heading southward
D) flock arrived from the north and is headed in a southwesterly direction
E) swans arrived from the north and is heading southwest

E : SWANS is plural can't use "is".
plural subject swans does not agree with the singular verb is.

C : corrected the SV but what about "Have"?
The singular subject flock does not agree with the plural verb have.

so the only option left is D.

white flock is singular so we need "is" hence D :flock arrived from the north and is headed in a southwesterly direction
