I went with B and bombed it. Is 'D' correct because it removed comma after 'problems' to link 'fraught process' to 'working together' and NOT to 'team finishing the work'?

I chose option B, and I think this is right choice.
"Working together, which was a process fraught with problems, the two" - the clause startng from which is providing additional information of the "working together" process. the main subject "two firms" must be set off from the rest of clause.

Please let me know what makes the option B wrong?
