"Recommended" is a futuristic reporting verb followed by the word "that." The next verb should be in its root form.
If you are talking about a hypothetical situation, "if were... + would" is correct.

A. had been below 5%, the board of directors would have recommended that all further investments must --> Incorrect. Option C seems to be more concise and clear than A.
B. was below 5%, the board of directors would have recommended that all further investments should --> Incorrect. For simple past the clause should begin with would instead of would have.
