That is a substantial improvement from last time!
680 is a respectable score. The median score range of 80% of the class of top schools would be 680 - 740.
4 and 5 are about ok scores in IR. They will probably not do anything for your chances - neither decrease them, nor increase them.
You can apply with this score to the top schools or you can choose to retake. It all depends on the timeline you have in mind. Do ensure that you have plenty of time to write very good essays. They will play a major role in the decision of the adcom.
