I know my verbal is weak and especially SC. So, I need some help. I am going to give GMAT again in 3 weeks and so it would be great if anyone can tell me what steps i should take to improve score by 100+ points. I am going to browse through this forum and read success stories as well. Thank you all

The first and the most important suggestion that I will give you is: Do NOT take the GMAT so early if you want a drastic improvement in your score. Going by the scores, I feel there is a problem in understanding the concepts that are tested on the test.
Also, since you would not be able to apply in the Round 1 application cycle, shifting your GMAT further by a month will cause no harm.
To improve your score, you should solve all the official problems and understand the basics. SC tests some basic principles such as Subject -Verb agreement, Comparison, Parallelism, Modifiers and usage of certain words. By mastering these, you can score good on the SC portion.
Did you attempt any mock CATs? If yes what where your scores on the tests?
There are still some universities which are till end of sept and early oct and I would like to apply to those or at least try. I gave one MGMAT and one GMAT prep. The score was 540 and 570 respectively.

Yes, there are still a number of schools with deadlines in early October, but 3 weeks from now makes the date as 28/29 This will not leave you with a time frame to finish your applications. Presenting a strong application will be a herculean task in such a short span of time.
Moreover, going by the scores, your are likely to score in the same range of your CATs. What is your target score?
For verbal, I recommend the Manhattan SC and CR strategy guides. Work through these diligently, solving problems at the end of each section after properly understanding the concepts. If not SC, you should be able to improve CR for sure with the strategies in Manhattan. For SC, focus on parallelism questions and common usages of terms (SC Strategy Guide). Also practice questions with the correct noun modified by the clause given - usually it is placed near the modifier etc. that kind of stuff.
Taking one prep pack test and one mgmat is not going to help you. Take more tests - they will give you better time-management skills and a better idea of various kinds of questions. Just for today, gmatclub has some tests available that are usually paid/accessible only after 25 kudos. Take advantage of that. For quant, gmatclub math guide (freely available) is a very good resource.
Hi! since you specifically mention SC, thought I will mention that our sentence correction book Sentence Correction Nirvana is perhaps the only book that offers a score improvement guarantee, and is especially designed for non-native speakers.
After reading the book twice (yes! it's an academic book, and so must be read twice in all seriousness, to reinforce the concepts), you will start looking forward to solving SC questions!
The book is available on Flipkart and Amazon.in. You might want to refer to these sites, to also read testimonials of how readers have benefited.
