All options ending in "required" are not correct because "required" requires (pardon my repetitiveness) a subject (noun or pronoun).

A. wheelchair access ramps that it had installed well before local law required
Incorrect, see reasoning above.

B. wheelchair access ramps that had been installed well before local law required it
Incorrect. Although meaning is okay, there is a solid problem with this option. Ramps is a plural noun, so there should be a pronoun or noun.

C. the installation of wheelchair access ramps well before local law required
Incorrect, see reasoning above.

D. having installed wheelchair access ramps well before local law required them
Correct. No pronoun ambiguity, clear meaning.

E. having installed wheelchair access ramps well before local law required
Incorrect, see reasoning above.

We do no have anything after required in C, thus creating meaning error. It is wise to leave out part of the sentence that is repetitive. We must however include words when there is some meaning error.
Full sentence would be " long before the law required them to be installed." here meaning is clear so we need them in D.

In D, what does "them" actually refer to?

IMO , the word "them" need to refer to the installation.
However, in D, it seems to refer to "wheelchair access ramps"

Please explain.

The 'them' has to refer to "wheelchair access ramps" and not the "installation" as the law required the "wheelchair access ramps"

I could not understand why is C incorrect. Also, why is 'them' in option D necessary. If not C, I would have chosen E

In 1990, a charitable foundation presented the local branch of a national chain of grocery stores with a humanitarian award for wheelchair access ramps that it had installed well before local law required.

A. wheelchair access ramps that it had installed well before local law required it has no clear antecendent
B. wheelchair access ramps that had been installed well before local law required it access ramps is plural hence it is wrong
C. the installation of wheelchair access ramps well before local law required the verb required needs a noun
D. having installed wheelchair access ramps well before local law required them
E. having installed wheelchair access ramps well before local law required the verb required needs a noun


Please correct me if I am wrong in the elimination reasons I have used.
