A)unsanitary conditions in 90 percent of them and they ranged
B)conditions in 90 percent were unsanitary and ranging

D)that 90 percent had unsanitary conditions ranging..................best possible choice
E)that 90 percent of them had conditions that were unsanitary;the range was

Hence, D will be the answer.

Last year a health department survey of 100 restaurants in Miami found unsanitary conditions in 90 percent of them and they ranged from unwashed utensils to expired raw ingredients.

The question tests the usage of tense, modifier and pronoun.

Meaning
Last year, a survey was done. According to the survey, 90% ofthe restaurants had poor sanitary conditions. The poor sanitary conditions ranged from unwashed utensils to expired raw ingredients.

Usually while presenting evidence such as the one in this case, found , it must be followed by that

POE
A- unsanitary conditions in 90 percent of them and they ranged
1.According to this option, there are two different findings from the survey. But the meaning implies only one finding. The sentence" Ranging from unwashed utensils to expired raw ingredients" adds a description to the finding.

2.Usually while presenting evidence such as the one in this case, found , it must be followed by that

B- conditions in 90 percent were unsanitary and ranging
the proposition in must be followed by some noun. Example, In the box, in the cabin
conditions in 90 % doesn't refer back to restaurants by any means

C- a range of unsanitary conditions in 90 percent of them.
This sentence changes the meaning of the sentence.
According to this sentence, the survey found a range of unsanitary conditions. It implies the unsanitary conditions are already known prior to the survey only the range was found through the survey.

D- that 90 percent had unsanitary conditions ranging
1.Usually while presenting evidence such as the one in this case, found , it must be followed by that
2. By changing range as a modifier ranging, the sentence clearly implies that there is only one finding the rest adds more information to the finding.
The usage of had can be justified. Since the survey is done last year, the prevailing sanitary conditions would have been there before the survey for a long time.
