You have a solid profile (with progressive work experience, good academics, and solid extracurricular involvement), and while the schools you've listed (particularly Kellogg, Haas, and Sloan) are very competitive, they're not out of range for you, though they might be a reach. Just make sure that you make it clear why each program is a strong fit for you based on your research. I would also consider Michigan Ross - their Tauber Program (geared towards those with a background in/passion for Operations) seems like it could be a good match for you. I would say Smeal is a fairly safe option, and UCLA and ISB are probably more in your target zone, though they are competitive programs as well.
