CONGRATS on that 760, outstanding!!
So you have a lot going for you...
-excellent GMAT
-strong work experience including recognition, progression & leadership
-lots of involvement/extracurriculars (note: the activities etc are basically reported on an honor system-- it is not necessary to have documentation/certificates!! Of course if a school ever found out someone was dishonest you could be kicked out etc

, but no stress on documenting it all)
-some volunteering
-CFA
Challenges....
-as you described-- you have a management degree. In general it can be QUITE tough to actually go for a 2nd MBA degree!! however.... my understanding is that the PGDM is somewhat different, and if you were not actually awarded an "MBA" and it was right after undergrad, and you can make a case for how it was NOT equivalent and why you DO feel you need this MBA.... you could still have a shot.
-not sure what your post-MBA goals are... keep in mind it's a good idea to have a vision you can clearly explain for your short and long term post MBA goals, and that these can often impact your admissions decision as well
All things considered your school list is likely reasonable, especially if the degree you received was NOT an MBA...
