Kind of a tough one; what are your goals? If you plan to do consulting or something involving on campus recruitment you are getting a little on the old side. So that would indicate R3. However optimally, round 3 is best for those with sky high GMATs or URMs. Most seats are taken and they are adding frosting on the cake at that point. Like someone to round out diversity or bump up the GMAT.
You might consider taking your top schools and applying to a handful now and then if now traction R1. But mix it up in terms of ranking because reapplying in R1 is not an option; too soon to have shown improvement. Other consideration would be January term for CBS given your background if you don't need an internship (but only if you truly dont.) January batch at INSEAD is good for IB, obviously consulting or if you want a one year program and need an internship.

Post-MBA Goals: Transition to healthcare strategy, preferably companies looking for non-traditional solutions to depression that would have been helpful in prior experience (i.e. Johnson and Johnson - newly approved Ketamine-based drug, companies looking at psilocybin).
