rex199124, what a super interesting story you have. I think you've got a great profile, with a few things to manage.
First, I'd say that your academic profile seems strong, although I do think a GRE may be a better score for you to submit if you can manage to get something near your previous score without too much effort. While a 720 is a nice score, at the tippy top schools, it's still below the current averages. Your GRE score however was very solid indeed. I'd love to see what you did with a retake there, assuming you had time/it isn't too taxing to execute.
Professionally, it sounds like you have some really good accomplishments that show how you performed well compared to peers, and I think you provide some profile diversity with your scientific background. That said, it sounds like you have only 3 real years of WE and are on the older end of the range of applicants...I think what I'd want to know more about is not just the reason for the periods when you weren't working but what you were doing in that time. Hopefully, the answer is: moving forward in some way and making yourself stronger. This will be easier to explain/show in some applications more than others (MIT has made this kind of explanation tricky in recent years, in my experience).
Personally, you have some fabulous leadership and impactful contributions. I really like hearing about your own connection to the cause you are so passionate about, and how you're applying that. I would press on your goals a little bit though, to strengthen your argument.
As for schools, you have a lot of names on there that are very difficult, but I think it's a good start. I might look at adding a few other names like Duke Fuqua and Michigan Ross, which have great healthcare programs.
