The underlined portion is explaining "some industries". It is a situation "in" some industries. So "in which" is better suited than "where". The problem with (B) is the use of "has" with plural subject "fundamental changes". Thus (D) is the correct answer.

where---incorrect cannot modify industries
changes---plural...so has not have

1st error : where vs in which
2nd error : S-V agreement, S - changes V - has/have

Correcting S-V agreement, we choose 'have'. So rule out options with 'has'. B is out.

'in which' is right. Because where can not modify industries. So rule of A, C and E. [A and C are identical, typo??]

Here only 2 thing we have to take care. "Where Vs which" and "and Vs or"

As per the context if something is weakened then we can not hurt that again. So the usage will be weakened...or hurt. Is is similar to an OG question where some cattles were injured or killed. (Same applies here injured & kills does not make sense).
B has SV agreement error too
In which correctly modifies industries.

The thing is "and" is not wrong here, rather the options which has "and" are wrong.
We cannot be sure of the intent of the author and therefore "and" and "or" are both correct in this context.

Let us look at the two options which has "and"

Of course makes sense

I was too stupid to eliminate all the choices with or and then eliminated B coz of "has"

The Official answer explanation also states that the correct connector is 'or' and not 'and'.
Could you please throw some light as to why 'or' is correct?

Responding to a pm:

You need to look for the best option among the given choices.

The have/has decision point is suitably dealt with in (A), (C), (D) and (E).

While referring to "some industries", the use of "in which" is certainly better than the use of "where" - which usually signifies a physical place. Only (D) uses "in which" and is the answer.
