A. Smoking as well as obesity are leading risk factors for heart attack Incorrect 'as well as' is used to introduce additives. therefore, verb should be coherent to smoking which is singular i.e. 'is'

My answer is (D). It took me 17 seconds.

(A) "Smoking as well as obesity" is singular. (Compare with "Smoking and obesity", which is plural")
(B) "Smoking and obesity" is plural.
(C) See (A)
(E) More wordy than (D). Also ambiguous. It can mean
EITHER
Smoking is the leading risk factor for heart attack and for obesity.
OR
Smoking as well as obesity is the leading risk factor for heart attack and for obesity.
