A) that if the H5N1 avian flu strain does begin to spread from person to person, antivirals are then sprayed over the affected geographical areas.
Bolded part is wrong here.

B) that if the H5N1 avian flu strain does begin to spread from person to person, antivirals be sprayed over the affected geographical areas
that clause perfect, antivirus be sprayed is according to subjunctive rule. Seems no error, keep it.

C) that antivirals should be sprayed over the affected geographical areas, if the H5N1 avian flu strain does begin to spread from person to person,
Antivirus should be sprayed is wrong here...

D) if the H5N1 avian flu strain does begin to spread from person to person, antivirals should be sprayed over the affected geographical areas
same error as C.

E) spraying antivirals over the affected geographical areas, if the H5N1 avian flu strain does begin to spread from person to person,
Complete disaster to sentence.

B is perfect answer here.
All bossy verbs such as recommend, mandate etc. use subjunctive mode in which that clause uses unaltered verb only.
