That were killed last year - past tense, So training of soldier - should be in past perfect tense to maintain the tense sequence. Moreover another error the sentence has: Killed & injured seems nonsensical, it should be killed or injured.
A. Some soldiers that were killed and seriously injured in the fighting last year were not
B. In the fighting last year some of the soldiers that were killed or seriously injured were not
C. Last year some of the soldiers that were killed and seriously injured in last year’s fighting had not been
D. Some soldiers that were killed or seriously injured in the fighting last year had not been - Correct Choice
E. Some soldiers that were killed or seriously injured in last year’s fighting have not been
Hence correct choice D

Killed and Seriously Injured is illogical!!
So A and C are gone
In the fighting last year soldiers were not trained also does not make correct sense. So B is out.
Had vs Have, Clearly we will use Had as it is clear the sentence is refering to the past event that happend and ended int he past.
Had Wins
E is out.
D is correct.
