The Leakeys discovered Homo habilis , the first tool user, with two teeth found at Olduvai Gorge in Tanzania.

IMO, the answer is (E).

A. Homo habiilis, the first tool user, with two teeth found - This distorts the meaning of the sentence.
B. Homo habiilis, the first tool user, with teeth that were found - This distorts the meaning of the sentence.
C. When two teeth of Homo habilis, the first tool user, were found - The construction is wrong and awkward.
D. Homo habilis, due to two teeth found of the first tool user - This seems like the Leakeys discovered Home habilis because of two teeth found of the user. This is not the intended meaning.
E. Homo habilis, the first tool user, when two teeth were found - This clearly implies the correct meaning of the sentence.

I doubt whether E makes any complete sense or for that matter, any of the choices can make the grade.

E. The Leakeys discovered Homo habilis, the first tool user, when two teeth were found -- Can we make out whose two teeth were found? It could be of Homo habilis or for that matter those of the Leakey's or just those of any other animal. Second, 'when' is not the correct diction in the context. It gives an impression that at the very moment of their sighting the two teeth, the discovery was made. Maybe the finding of the teeth was the beginning of a series of several other such finds but not the end in the least. Therefore, logically E is anomalous

The first tool user, in this sentence modifies Homo habilis.
