Chose A over B

Thought 'where' seems to modify the whole clause before it(breaking the touch rule); seemed less awkward than 'in which'
Would a comma before 'where' would have made a difference ?

There is no way we can allow "in which" to jump over the entire preceding clause to modify the subject "sites." I don't think you'll find a single official question that allows this. Also, there's not enough reason to eliminate A. We can't say it's wrong that all those things happen in India, which is after all the site of action. In any case, a proper SC question will have difference at the ends of the answers, not just one difference at the beginning.

Another poorly constructed question in my view.

Not saying B is wrong, but there is no ground to eliminate A.

I understand the difference between "where" and "in which" here. But the option E, is it not an example of noun+noun modifier? Where the part "an exchange.." is referreing to the entire preceeding clause?

In such cases, I would (at the very least) expect a comma after the clause (after "India" in this case).
