How is answer "A" correct (I think all of the answers should be wrong). The reason I say this is because of the pronoun "she". Shouldn't it be unclear whether "she" refers to Sally or her roommate?

To check if pronoun error is there or not, i think of the following framework: put the noun in place of pronoun and see if the sentence still makes sense.

so lets do that here:

Sally found her roommate’s bed unused,
so Sally figured that "she" Sally's roommate must have slept over at Steve’s house.

OR

Sally found her roommate’s bed unused,
so Sally figured that "she" Sallymust have slept over at Steve’s house.

The second doesn't make sense since sally found as per the given framework, hence sentence doesn't have pronoun error.
