The ratio of the percent paid by the patient to the percent paid by the patient’s primary insurance was greatest in the year and least in the year .
In a ratio if numerator is increased and denominator decrease new ratio is more than the old ratio and vice versa.
By looking at the figures we can see for 2013 % paid by patient across all years is highest and similarly % paid bt primary insurance lowest. this gives the fisr answer

In 2008 % paid bypaitient's is lowest and % paid by primary insurance highest . So this gives us the lowest ration

If the numerator is greater than the denominator then the ratio will be greatest, else least. From given options, ratio is greatest for 2013 and least for 2008.
