Could anyone potentially clarify why the position of "for example" is wrong in option D?

A semicolon separates two main clauses, and placing "for example" before semicolon makes the first clause incomplete - "for example" should be after the semicolon.

Yes, it is there - in the original sentence, the comma is outside the underlined portion. (Though this may be a bit difficult to see in the original sentence, option A confirms this.)
